Costs and Insurance


The majority of clients elect to bill insurance for their sessions. As a result of this, we require a copy of the insurance card on file. If you choose not to bill insurance, and would prefer to pay out of pocket, we offer a 15% discount on our rates for cash-pay. Please call 541-343-7200 for additional information.

In-Network

In order to bring our clientele quality services in-network with their health insurance, Champion Family Therapy is contracted with several major insurance providers.

At this time, we are in-network* with:

 
 
  • Pacificsource

  • Regence/Blue Cross Blue Shield

  • Moda

  • Anthem

  • Pacificsource OHP/Community Solutions Medicaid

  • Providence (in-network beginning July 1, 2023)

 
 

*Though we do have contracts with each insurance company listed above, every insurance plan is unique. It is possible your plan may have limitations that are not known to this practice prior to the start of services. Your insurance provider will be best equipped to assist you in understanding what your specific plan covers, as well as what costs you will be responsible for.

Out-of-Network

We do not bill insurance for out-of-network services, however, we do offer out-of-network services for those who want them. For clients’ with out-of-network coverage, we require payment from the client at the time of service. We are happy to provide a superbill to clients who wish to seek personal reimbursement from their insurance provider.

Please be aware Trillium OHP does not currently contract with Champion Family Therapy and does not allow non-contracted providers to serve their members. If you have Trillium coverage, we are happy to provide referrals to help you get connected with a contracted provider. Email our office at info@championfamilytherapy.com, or contact Trillium directly for referrals. Effective January 1, 2020, Champion Family Therapy is contracted with the Medicaid plan offered by PacificSource in Lane County.

Additionally, are unable to offer services to clients with Medicare or TRICARE (including TRICARE West) coverage due to legislative limitations.
