For prospective clients: Please do not complete new client forms prior to completing our new client submission form. If we have not received a new client submission form, you have not been added to our waitlist.

Please note: Completion of new client forms, does not guarantee our practice is able to serve you. Our clinicians have an ethical obligation to practice within their scope of practice and to only provide services determined to be clinically indicated. If our practice is a good fit for your presenting concerns, having your forms completed will expedite the scheduling process.
